Now in our fourth decade, we continue to dream of a world where no child lacks access to safe, affordable surgical care. We are proud to say that in 2024 we accelerated our efforts toward closing the gap between where our patients live and access to the essential surgical and comprehensive care they need. As a locally led, globally supported organization, we are focused more than ever on education and investing in local surgical teams.
No matter where we travel around the world, our patients and their families continue to amaze us with their bravery and their dignity. We could share hundreds of stories of courage, but we hope you will enjoy reading about Adolphe in Rwanda and the story of Mateo in Mexico, both of whom made multi-hour journeys seeking care. We are steadfastly committed to the need to bring essential surgical and comprehensive care closer to their communities.
For Operation Smile, education and training have always been part of our DNA. In fact, we say it is our force multiplier. But creating the best training environments means delivering mentorship at the local level – onsite to local health care providers. We are grateful for the support of our long-term partner, Johnson & Johnson, who sponsored a series of global Nursing Leadership Trainings reaching over 200 nursing leaders from more than 25 countries. Backed by our Medical Global Standards, these trainings equip nurses to better address not only cleft conditions, but also other essential emergency and surgical needs.
Our Policy and Advocacy Team works tirelessly around the world to promote greater access to cleft care and surgical care through cross-sector planning with governments, academia and the health care sector. After two years of working in support of the National Surgical, Obstetric and Anesthesia Plan (NSOAP) in Ghana, we celebrate that in September, a milestone was reached when the Ministry of Health of Ghana launched the country’s first-ever NSOAP. Read about how it will serve as a strategic roadmap for surgical care delivery throughout Ghana.
While we are proud of our past, we recognize that by harnessing the power of artificial intelligence, we can step into the future to ensure that our health care professionals provide gold standard care, irrespective of even the most resource-limited settings. Through a partnership with Microsoft’s AI for Good initiative, we piloted a groundbreaking tool and technology that allows our surgical teams to analyze outcomes in an unbiased and standardized way. Operation Smile has always sought collaborations with transformative partners, and we have never been afraid to think big. We hit the jackpot with our recent partnership with Beast Philanthropy, the nonprofit arm of global YouTube creator MrBeast. You will read about their video from our program in Mexico that has reached nearly 40 million views. Visibility at this scale multiplies our efforts in ways we could have never imagined!
Youth empowerment is built into the foundation of Operation Smile. We celebrated our 32nd annual International Student Leadership Conference at Emory University in Atlanta, Georgia. We love spending time with these enthusiastic advocates who share a passion for becoming agents for change. Thanks to the support of Align Technology, we were able to welcome even more deserving students through scholarships who joined the hundreds of like-minded young people who believe in the power of advocacy and action.
Corporate partnerships are not just connections with companies – they are connections with people. We could not be more pleased to highlight Mediterranean Shipping Company and their passionate employees, a proud partner since 2011. MSC USA offices showed their their competitive spirit through numerous golf outings and events that rallied hundreds of golf enthusiasts, MSC vendors and customers nationwide each year, raising collectively more than four million dollars to transform the lives of children around the world.
Our collaborative efforts throughout 2024 propelled us forward in our four-decade-long commitment to advancing health equity to ensure no one is left behind. 2024 Program Highlights
At Operation Smile, we are committed to bridging the gap in access to essential surgeries and health care, starting with cleft surgery and comprehensive care. By addressing barriers such as distance, financial constraints, limited health infrastructure, and shortages of trained professionals, we are working to bring high-quality, life-changing care closer to the communities we serve.
In collaboration with local governments, nonprofits and health systems, and supported by our generous donors and corporate partners, we are creating sustainable solutions that strengthen health systems and expand access to care. In the 2024 fiscal year* Operation Smile’s clinical programs reached new heights, expanding essential services to more patients across the globe.
The following highlights from fiscal year 2024 reflect the hard work and dedication of the volunteers, employees and supporters who make this possible. Through their efforts, we can continue to deliver safe, high-quality cleft care to those who need it most.

204 Surgical Programs
Operation Smile conducted 204 surgical programs in 31 countries across Central and South America, the Caribbean, the Middle East and North Africa (MENA), Sub-Saharan Africa (SSA) and Asia.
86,000+ Patients Received Surgical & Comprehensive Care
We provided over 86,000 patients with surgery and comprehensive care through medical programs, care centers and partner hospitals. Of these patients, more than 60,000 received comprehensive services including dental care, speech therapy, nutrition and psychosocial support, including 15,517 patients who received surgical care. Approximately 20,000 received oral health treatment in areas where access is limited. Since our founding in 1982, Operation Smile has helped over 500,000 people receive life-changing surgical procedures and comprehensive care.
60+ Ongoing Comprehensive Care Locations
Beyond our surgical programs, Operation Smile provided patients with ongoing comprehensive cleft treatments, including dental care, speech therapy, psychosocial care, nutritional support and more at over 60 locations across 30 countries through our hospital partners and care centers.
6,500+ Health Care Workers Trained
Operation Smile made a substantial investment in health care capacity by training over 6,563 health care workers through both operative and non-operative programs. Training and education are vital, equipping local providers with the skills needed to deliver high-quality care and expand access to care within their own communities.
Hospital Infrastructure and Equipment Investments
We made hospital and equipment investments in countries, including the Philippines, Rwanda, Madagascar, Morocco and Peru. These investments are increasing surgical capacity, enhancing training opportunities and laying the foundation for continuous care beyond
surgical programs.
Medical Oversight and Quality
Operation Smile is committed to ensuring that every patient receiving care through our programs and partnerships benefits from the highest standards of safety and quality. Building on our long-standing Medical Global Standards and commitment to continuous improvement, we launched the Four Pillars of Safety Checklist. This tool supports consistent, high-quality care across our 95 hospital partners and 204 surgical programs, reaching 15,517 patients.
Expanding Access to Care and Bringing it Closer to Home
At Operation Smile, we believe every child deserves access to timely, safe and effective cleft care—no matter where they live. Our work goes beyond surgery to support patients and families throughout their health journeys, investing in the health workforce and strengthening local health systems to create lasting impact. Adolphe’s story is one example of how we’re creating healthier futures and bringing care closer to home for families in Rwanda.
Born in the hills of northwestern Rwanda, Adolphe is the first child of his mother, Nikuze. When she gave birth, she was overwhelmed with concern. Doctors informed her that he had been born with a cleft lip and palate, a condition that could affect his ability to eat, drink and even smile. “No mother wants to be told their child can’t smile,” she said. “As a mother, I love him, but it’s been difficult to witness how people look at him and treat him.”
In Rwanda, cleft conditions pose a significant challenge for families like Adolphe’s. While the procedure to treat a cleft can take as little as one to two hours in the hands of trained professionals, the country faces a shortage of surgical teams, particularly in rural and remote areas. This gap in access means that families often wait months or even years for the care their children need.
Adolphe’s journey to care was featured in the mini-documentary “Beyond the Code,” produced for Operation Smile by Partnership on AI and BBC StoryWorks Commercial Productions. The film follows the pilot of a new AI-powered platform developed in collaboration with Microsoft, designed to support surgical teams and training in low-resource settings.
When Adolphe was about six months old, his family learned about a surgical program supported by Operation Smile at Ruhengeri Referral Hospital. After a comprehensive medical screening with Dr. Faustin Ntirenganya, a reconstructive plastic surgeon with Operation Smile in Rwanda, Adolphe was approved for surgery.
The result was life-changing. “I was excited to see my son after the surgery without the cleft lip,” said his father, Josue. “I can’t thank the doctors enough for what they did for us today,” added Nikuze. “ Our child will be drinking, eating and smiling without any issues.” Now, the only sign of his surgery is a fine line of stitches that will fade over time.
Adolphe’s story reflects the importance of building sustainable health systems that make surgery more accessible. By increasing access to care, we’re helping ensure that more children like Adolphe receive the chance to grow up healthy, confident and inspired to smile.

Investing in Nursing Leadership to Strengthen Health Systems
At Operation Smile, we believe that investing in local health care leadership is essential to building capacity and strengthening health systems. Sustainable change begins with investing in people. Education acts as a force multiplier, equipping individuals with the knowledge to enhance their skills and extend their impact to others in their communities. With the support of our long-standing partner Johnson & Johnson, we advanced this commitment in 2024 through a global series of Nursing Leadership Trainings held in Sub-Saharan Africa, Latin America and the Carribean, and Asia.
These trainings equipped 200 leaders from over 25 countries with advanced education in perioperative care, patient safety and leadership. Each program combined hands-on learning with mentorship and collaborative project work, offering participants the opportunity to deepen their expertise in pre-, peri-, and post-operative cleft care, global medical standards and pediatric perioperative nursing. In regions where standardized training remains limited, these sessions provided nurses with the knowledge and tools to deliver high-quality care and support stronger, more resilient health systems.
As part of the training, each nurse leader developed a Capstone project tailored to the specific needs of their local health system. From individual efforts to group initiatives, these projects—guided by Johnson & Johnson mentors—are already making an impact: improving clinical practices, expanding training opportunities and enhancing excellence in patient care.
Many participants have gone on to lead local training and workshops, multiplying the impact of their learning and reinforcing care delivery in their communities. These efforts are a critical part of expanding our global footprint in a way that is both effective and locally driven.
These programs reflect Operation Smile’s long-term commitment to health system strengthening through the training and mentorship of nurses who are driving meaningful, lasting change where it’s needed most—increasing access to surgical care that can be sustained by local health workers.

A Strategic Roadmap: Ghana’s National Surgical, Obstetric and Anesthesia Plan
A major milestone in 2024 was the launch of Ghana’s National Surgical, Obstetric and Anesthesia Plan (NSOAP) — a groundbreaking policy achievement that reflects that reflects the country’s commitment to expanding access to safe, timely and affordable surgical care.
The development of Ghana’s National Surgical, Obstetric, and Anesthesia Plan (NSOAP) was led by the Ministry of Health through a collaborative, multi-stakeholder process. Operation Smile and Operation Smile Ghana were honored to be recognized as key technical experts in this effort, working alongside the NSOAP Technical Working Group, the Ghana College of Physicians and Surgeons, the Ghana College of Nurses and Midwives and the Ghana College of Pharmacology, among other valued partners. This comprehensive plan is designed to strengthen the country’s surgical system and ensure that all Ghanaians have access to high-quality surgical, obstetric and anesthesia care. It also integrates these services within the broader framework of emergency, critical and operative care.
The NSOAP serves as a strategic roadmap for improving surgical care delivery across the country. Without such a plan, efforts to expand access can become fragmented — limiting their reach and impact. By coordinating interventions and setting clear priorities, Ghana’s NSOAP lays the foundation for a more effective and equitable health system. This effort underscores our commitment to partnering with governments and local health systems to expand surgical care, build capacity and deliver long-term solutions.
Throughout this process, stakeholders from government, academic, professional bodies, civil society and the health sector came together to assess gaps in surgical care, identify solutions and define measurable goals. The resulting plan takes a comprehensive health systems approach, targeting all six building blocks essential for resilient and equitable care. It focuses on strengthening service delivery, expanding and training the health workforce, improving health information systems, ensuring access to essential medicines and surgical supplies, reinforcing governance and leadership and securing sustainable financing. These efforts are critical to delivering high-quality surgical care that leaves no one behind and integrates seamlessly into the broader health system.
This initiative not only holds national significance but also sets a powerful example for other countries in the region and around the world. It demonstrates the impact of collaborative, cross-sector planning and reinforces the importance of local ownership and leadership in achieving health equity.
We are proud to have partnered with the Ministry of Health, the Ghana College of Physicians and Surgeons and other stakeholders throughout this process — contributing our expertise as part of a collective effort committed to creating lasting change. Ghana’s NSOAP marks a transformative step forward, and with its successful implementation, millions of lives stand to benefit.

Harnessing Artificial Intelligence to Advance Surgical Care
Operation Smile is harnessing the power of artificial intelligence to advance cleft care and build local health care capacity for patients around the world. Through a partnership with Microsoft’s AI for Good initiative, we’re piloting an innovative tool that enhances how our surgical teams assess, plan and evaluate cleft surgeries — with the goal of helping health care professionals provide gold standard care, even in the most resource-limited settings.
This groundbreaking work was spotlighted in Beyond the Code, a branded film series launched in October 2024 by Partnership on AI (PAI) and produced by BBC StoryWorks Commercial Productions. The series aims to uncover the ways that AI is impacting people’s lives and contributing to a more sustainable environment.
Filmed in Musanze, Rwanda, the story follows Adolphe, a young boy born with a cleft lip and palate, and his journey to receiving life-changing surgery. Through his experience, the film explores how AI is supporting our work by helping us provide consistent, high-quality care in under-resourced communities.
The pilot technology uses advanced algorithms to assess the severity of cleft conditions and evaluate surgical outcomes objectively. By analyzing tens of thousands of patient photos each year, the AI helps clinicians track outcomes and identify trends beyond what teams could do manually. It also supports surgical teams in setting realistic expectations with families by estimating the number of surgeries needed, recovery time and treatment complexity.
“Much of plastic surgery is subjective,” says Dr. Caroline Yao, plastic surgeon and SVP of patients and evaluations at Operation Smile. “Every surgeon may give you a different opinion about what they think of the result. But an objective answer we get from AI can bring all the practitioners’ opinions together.”
This innovation is not only transforming how we support health care professionals in providing cleft care — it’s also strengthening local health care systems. In addition to aiding evaluation, the tool supports training and education by identifying emerging talent in cleft care, especially in places where specialized surgical expertise is limited. By helping identify potential cleft surgeons who can serve as educators and mentors, the tool strengthens local expertise and amplifies the impact of our volunteer network. This multiplier effect in training and education helps entire regions benefit from sustainable, high-quality care.
This initiative reflects our broader commitment to health equity: making high-quality cleft care accessible to every child, no matter where they live. With something as simple as a phone or camera, providers in remote areas can receive assessments and expert guidance, helping to standardize care, expand local networks and bring surgery closer to home.

Amplifying Impact Through Strategic Partnerships
Operation Smile is committed to bridging the gap in access to essential surgery and health care, starting with safe surgical and comprehensive care that prioritizes patient needs and long-term outcomes. In 2024, we partnered with Beast Philanthropy, the nonprofit arm of global YouTube creator MrBeast, to amplify the stories of patients and families and share the impact of our work with millions around the world, raising global awareness of the need for safe, high-quality care and the importance of community-driven solutions.
In November 2023, the Beast Philanthropy team including Executive Director Darren Margolias joined us in Puebla, Mexico, during a weeklong surgical program. There, they met patients and families—many of whom had traveled long distances for the opportunity to receive care. Over the course of the program, more than 100 families were screened and treated thanks to the collaboration of local and international volunteers working to expand access to essential health services.
As part of the partnership, Beast Philanthropy produced a powerful video 100 Kids Smile for the First Time, bringing the stories of patients and families to a global audience. Within 24 hours, the video had been viewed more than 2.5 million times. Today, it has reached over 39 million views and continues to bring attention to the barriers families face in accessing care.
As Mauricio Rojas, executive director of Operation Smile Mexico said, “This partnership has been a transformative force for Operation Smile Mexico. It has opened countless doors to new allies and donors in the country, amplified our visibility, and allowed more patients to learn about our work and services.” He added, “But perhaps the most heartwarming impact of all is how it has fostered a sense of community among patients with cleft lip and palate, letting them know they are not alone in their journey.”
In recognition of its impact, the collaboration received a Silver Anthem Award in the Health Partnership or Collaboration category—selected from more than 2,300 entries worldwide. The award highlights how strategic partnerships can drive awareness, mobilize resources and help build stronger health systems rooted in equity and inclusion.

Empowering Future Leaders: International Student Leadership Conference 2024
Operation Smile’s International Student Leadership Conference (ISLC) is a transformative experience that equips young changemakers ready to make their mark with the skills to create meaningful impact in their communities. ISLC 2024, hosted at Emory University in July in Atlanta, Georgia, brought together more than 350 high school and college participants and chaperones from 28 countries for a week of leadership development, skill-building and global connection. Now in its 32nd year, ISLC remains a cornerstone of Operation Smile’s commitment to youth empowerment, cultivating a global community of student volunteers focused on improving health outcomes for people affected by cleft conditions.
United by a shared passion for service, these student volunteers spent the week honing their skills, exploring their potential and forging lasting connections with like-minded peers. Guided by the theme “The Power Within,” participants engaged in leadership workshops, interactive sessions and insightful discussions led by Operation Smile staff, volunteers and fellow students. From global health and advocacy to empathy and personal growth, each session equipped attendees with the tools and confidence to drive change in their home communities.
Thanks to the generous support of Title Sponsor Align Technology, ISLC 2024 provided a dynamic platform for participants to engage with motivational speakers, attend workshops and build lasting friendships. Align Technology’s support helped remove financial barriers, enabling a more diverse group of student leaders representing countries across Africa, Asia and Latin America to participate through scholarships and program funding—supporting a new generation of advocates for health equity. Align Technology also provided scholarships for 20 students born with a cleft condition.

For more than 30 years, ISLC has been a springboard for changemakers dedicated to compassion, service and equity in health care. Transforming Lives Through Locally-Led Care
For over 42 years, Operation Smile has been dedicated to transforming lives and communities by providing access to high-quality surgical and comprehensive care in low- and middle-income countries. We believe that everyone deserves access to safe, quality care—no matter where they live.
Mateo’s story is a reflection of this mission. Born in Huajuapan de León, Oaxaca, Mexico, Mateo was diagnosed with a cleft condition during a prenatal ultrasound when his mother, Francisca was four months pregnant. Though saddened by the diagnosis Francisca remained hopeful—encouraged by the belief that early surgery would help give her son the best possible future.
She held onto that hope as she prepared for the journey to one of Operation Smile’s surgical programs in Puebla, Mexico. Like many families seeking care, Francisca faced uncertainty, but she found reassurance in knowing Mateo would be in expert hands. “Before the surgery, I was distressed but calm at the same time,” she shared. “I knew my son was going to be well cared for.”
Mateo’s experience highlights the barriers many families face in accessing essential health care—and the power of locally led, comprehensive programs to overcome them. Our commitment goes beyond surgery — through speech therapy, nutrition support, oral health care and psychosocial services, we strive to support patients throughout every stage of their journey.

As a family company that shares Operation Smile’s values of finding sustainable solutions and lasting impact, MSC is a celebrated corporate partner for Operation Smile. Rooted in a trusted relationship built for over a decade, Operation Smile is proud to recognize MSC as a maritime and transportation industry leader committed to changing lives for those in need. MSC has delivered critical cargo, compassion and life-changing care to thousands of children around the globe through its company’s network of logistics and strategic support.
Over the years, MSC USA employees have joined the Operation Smile global family of volunteers and supporters. Additionally, MSC USA offices host annual golf outings throughout the U.S. that rally MSC vendors, partners and customers each year. To date, these benefits and events have raised collectively more than four million dollars and continue to leverage additional industry partners to join Operation Smile, directly supporting safe surgery and related medical care for thousands of children with cleft conditions around the world.
In April 2024, for instance, MSC and Metro Group colleagues traveled to Santo Domingo, Dominican Republic as non-medical volunteers, where they were fully immersed in surgical team activities to provide on-site care and support. From helping children and families feel comfortable in the waiting areas, to observing transformational cleft surgeries, these MSC representatives were volunteers and team members who made a significant impact on the lives of others. Thanks to a total team effort, Operation Smile Dominican Republic provided life-changing cleft surgeries for 92 patients in one week.
